A Family Information Form/Emergency Contact Information System is being developed for site users. FIF is attached below for your use to ensure that someone is able to contact you at all times in the event of a minor incident such as an unlocked door, or a major situation such as an area-wide emergency. All residents should complete the form and give a copy to a trusted neighbor, local relative or close friend. You may also choose to send the completed form to SSP for inclusion in a secure database to facilitate emergency notification or contact.

Household Inventory Forms are also attached for your use. A copy should be kept in a secure place away from your home to ensure that records are not lost in the event of fire or theft. SSP will not maintain an Inventory Database, but will consider special requests due to unusual circumstances.

A complete inventory is time-consuming, but invaluable in a time of crisis. Insurance and security specialists also recommend taking photos or video of each room in your home.
